Spoilers for the film below
The choice to have someone who had a grotesquely tragic battle with anorexia (in a film about this no less) be represented by Barbie seems like it'd be really tasteless, and it is. But, and I'd say as a result of this tastelessness rather than despite it, the film portrays Karen in a compassionate, careful, complex way. Her family actually got super mad about the film (which I'd actually say portrays them with similar complexity and compassion) and had all copies (except one held by a museum in New York) destroyed due to a lawsuit. Only recently has the film started to resurface.
Interestingly, the film was directed by Todd Haynes, who recently directed the critical smash Carol.
