President Ilham Aliyev has signed a decree to simplify the visa application process for visitors to the Eurovision Song Contest in May. Under the terms of the decree, signed on 10 February, visitors to the song contest from participating countries will be able to obtain a visa at any of Azerbaijan’s international airports.
To be eligible to apply for a visa on entry to an Azerbaijani airport, an applicant must have one of the following:
1) an official invitation letter issued by the Organizing Committee of the Eurovision Song Contest-2012
2) a document confirming accreditation in accordance with the relevant rules of the European Broadcasting Union (EBU)
3) a ticket for the Eurovision Song Contest-2012 or a document confirming the acquisition of a ticket
Applicants in the aforementioned categories have to apply for their visa within the following dates:
1) Vistors with an official invitation letter issued by the Organizing Committee of the Eurovision Song Contest-2012 and participants and members of delegations accredited in accordance with the relevant rules of the EBU – from 15 April till 1 June 2012
2) journalists and members of fan clubs accredited in accordance with the relevant rules of the EBU and ticket holders for the Eurovision Song Contest – from 10 May till 1 June 2012
The Cabinet of Ministers has been instructed to resolve logistical issues arising from the issue of visas at airports, while the Foreign Ministry is to publicize the procedure on its website and through its embassies and consulates. Outside of Eurovision, visitors to Azerbaijan have to apply for visas at the nearest Azerbaijani embassy or consulate, with the exception of residents of a country which has direct flights to Azerbaijan but no embassy.

Registration form for the foreigners visiting Nagorno-Karabakh is the only document confirming the fact of their visit to that fascinating land. There are no stamps in the passport . In fact, the Azeri authorities can not know whether you visited Nagorno Karabakh or not.

here's the form

Qele_Armenia, if a person who visited Nagorno Karabakh region has no stamp in passport then this person will have no problem in entering Azerbaijan unless he/she makes this illegal visit to Nagorno Karabakh region publically known.
